Context

A major growth corridor with a mix of younger families and older populations with complex health needs.

Key challenges include:

  • Higher-than-average rates of dementia in some areas

  • Elevated alcohol and other drug-related harm

  • Pressure on transitions between primary care, hospital and aged care

What we are doing

  • Mapping journeys of families and individuals with complex needs

  • Identifying key points where care coordination breaks down

  • Co-designing solutions to improve transitions and service integration

  • Building on existing integrated care and community-based models

Future work

As the program of work progresses in the South East region, updates will be provided on what the needs analysis finds, what we learn from patient journey mapping and the interventions that are developed to help reduce fragmentation.
