Meet the Team
The Transform Collaboration
Transform brings together a diverse group of experts and organisations committed to improving healthcare for people with chronic illnesses. Our collaboration includes:
People receiving care
Chief Investigator and Associate Investigators from the University of Melbourne and Monash University
16 healthcare research and healthcare organisations across Melbourne
Our Team:
A Diverse Mix of Voices
We believe that the best solutions come from listening to everyone involved in healthcare. That's why our key stakeholders include:
Consumers*: People living with chronic illnesses and their carers
Clinicians: Doctors, nurses, and other healthcare professionals
Health Services: Organisations from across the healthcare system
Healthcare Funders: Those who finance and support healthcare initiatives
Researchers: Experts in Implementation Science, Health Care Systems and Translation
Health Economists: Research specialists in healthcare economics and policy
Our Approach:
Co-creating Solutions
By bringing together this diverse group, we ensure that:
Solutions are centred around the needs of people receiving care
Multiple perspectives are considered from persons receiving care, clinicians, health organisations, and other stakeholders
Solutions are created collaboratively
Those who will implement and benefit from the changes are involved from the start
This inclusive approach, driven by the patient journey experience, helps us develop practical, effective solutions that will work in the real world, benefiting both patients and healthcare providers.
*We recognise the term ‘consumers’ is not universally accepted, and we are working collaboratively to determine a suitable alternative, guided by the Australian Commission on Safety and Quality in Health Care (https://www.safetyandquality.gov.au/faqs-about-partnering-consumers-nsqhs-standards-second-edition). Currently we are using the word interchangeably with ‘people with lived experience’, ‘people receiving care’, and ‘patients and carers’.
