Key challenges include:
High rates of alcohol and other drug use
Increasing mental health presentations to emergency departments
Preventable hospitalisations related to chronic illness
What we are doing
Mapping journeys of families and individuals with complex needs
Identifying key points where care coordination breaks down
Co-designing solutions to improve transitions and service integration
Building on existing integrated care and community-based models
Future work
As the program of work progresses in the Eastern region, updates will be provided on what the needs analysis finds, what we learn from patient journey mapping and the interventions that are developed to help reduce fragmentation.
