North West Region
Context
North West Melbourne is a rapidly growing region with a high proportion of children, young people and families, alongside increasing demand from an ageing population.
Key challenges include:
Higher levels of social disadvantage and family violence
Increasing rates of low birth weight
Significant population growth and service demand
What we are doing
Mapping journeys of families and individuals with complex needs
Identifying key points where care coordination breaks down
Co-designing solutions to improve transitions and service integration
Building on existing integrated care and community-based models
Future work
As the program of work progresses in the North West, updates will be provided on what the needs analysis finds, what we learn from patient journey mapping and the interventions that are developed to help reduce fragmentation.
